Résumé

Une discordance existe dans la littérature au sujet de l'utilisation des évidences radiographiques de la gonarthrose pour l’évaluation de la pathologie. En effet, les études qui se sont intéressées à la corrélation existant entre grades radiologiques de la gonarthrose et scores fonctionnels obtenus par questionnaire auto-administré par les patients se sont soldées de conclusions très variables : alors que pour certains, la gonarthrose avancée est synonyme de perte de function importante, d’autres ne trouvent aucun lien entre les deux observations. À la lumière de ces résultats, le diagnostic radiologique seul ne permettait donc pas d’expliquer la fonction du genou. Une mesure quantitative de la fonction articulaire peut être considérée en complément. Plusieurs méthodes d’analyse de la cinématique 3D de l’articulation fémoro-tibiale par imagerie médicale existent. Toutefois, aucune ne permet d’évaluer le genou en charge par le biais d’une mesure fiable (répétabilité et précision) tout en réduisant le préjudice subi par le sujet (exposition à des radiations ionisantes, protocole invasif). L’objectif principal de cette thèse est donc de développer et de valider une méthode d’analyse de la cinématique 3D du genou en charge répondant à ces problématiques. Ensuite, il s’agit de vérifier si l’évaluation radiologique de la gonarthrose enrichie d’une mesure cinématique 3D au genou est plus fortement corrélée au score fonctionnel.

Dans la première partie du travail, une méthode de calcul de la pseudo-cinématique fémorotibiale a été développée. Cinq flexions du genou ont été imagées avec le système radiographique biplan basse dose EOS® (0, 15, 30, 45 et 70°) ; la paire de radiographie en extension totale a permis la reconstruction 3D du fémur, du tibia et du péroné. Après la segmentation des contours osseux sur l’ensemble des radiographies, un recalage 2D/3D intrinsèque basé sur l’iterative closest point a permis d’extraire la position relative du tibia par rapport au fémur, d’un angle de flexion au suivant. L’introduction d’un recalage pondéré graduel et du recalage du corps rigide tibia/péroné a permis de diminuer considérablement les erreurs d’estimation des rotations axiales. À l’issue d’études de validation sur dix sujets, l’erreur de mesure moyenne associée à la répétabilité et à la précision est inférieure à 0.4mm et 0.5° et celle due à la sensibilité de la méthode à la reconstruction 3D inférieure à 1.7mm et 1.4°. L’étude de l’adduction (ADD) pseudo-cinématique par analyse de regroupement a permis d’isoler ce paramètre comme un indice radiographique fonctionnel de la gonarthrose : tandis que les asymptomatiques se trouvaient en ADD modérée (<5°) ou en abduction (ABD), les sujets gonarthrosiques tendaient à être en ADD sévère supérieure à 5° (14 patients atteints d’arthrose du compartiment interne du genou, 14 sujets asymptomatiques). En considérant que l’ADD du genou est un facteur de risque pour l’apparition et la progression de la gonarthrose, il a été possible de calculer des grades Kellgren-Lawrence composites pour 28 sujets. Les grades Kellgren-Lawrence originaux ont ainsi été augmentés par l’ADD sévère, diminués par l’ABD. La corrélation des scores fonctionnels KOOS (Knee Ostearthritis Outcome Score) avec ces grades composites s’est révélée légèrement mais non statistiquement supérieure à celle avec les grades Kellgren-Lawrence originels. Dans une dernière partie, le screw-home mechanism pseudo-cinématique a été évalué (7 patients atteints d’arthrose du compartiment interne du genou, 5 sujets asymptomatiques). Alors que les tibias asymptomatiques se trouvaient en rotation externe (9° en moyenne), les tibias arthrosiques se trouvaient en rotation externe plus faible (0-5°). Pour un sujet asymptomatique, un faible ratio des pentes sagittales des plateaux tibiaux et une faible pente sagittale du plateau tibial médial ont été mis en lien à la rotation tibiale interne et des résultats suggéraient l’implication des pentes coronales des plateaux tibiaux sur la rotation externe du tibia.

À l’issue de ces travaux de doctorat, une solution technique a été fournie pour l’évaluation précise et répétable de la pseudo-cinématique 3D du genou asymptomatique et gonarthrosique à l’aide du système EOS®. De plus, l’adduction a été isolée comme marqueur fonctionnel radiologique de la gonarthrose et a permis de confirmer l’intérêt à compléter l’évaluation radiologique de la gonarthrose par une telle mesure. L’analyse de la morphologique du genou associée à celle de la fonction a en outre révélé des pistes vers une meilleure comprehension de l’articulation, une évaluation plus complète de la pathologie et plus représentative de la perception qu’en a le sujet atteint.

Functional evaluation of the arthritic knee by studying the squat pseudo-kinematic

In the literature, there is discordance about the use of arthritis radiological evidence for the evaluation of the pathology. Indeed, the studies that evaluated the correlation between the radiological grades and the function scores calculated from self-reported questionnaires obtained highly variable conclusions. When for some, severe knee arthritis correlates with an important function limitation, others did not find any relation between the two observations. From these results, it appears that radiological diagnostic alone can not explain the knee function. A quantitative measure of the articular function can complement the arthritis diagnostic. Several methods are accessible to analyse the 3D kinematics of the femoro-tibial joint with medical imaging. However, none allows the evaluation of the weight-bearing knee from a reproducible and accurate measurement, while minimising the prejudices the subject undergoes (exposition to ionizing radiations, invasive protocol). Hence, the main objective of this Ph.D. dissertation is to develop and validate a method to analyse the kinematic of the weight-bearing knee, with a solution to the problematic mentioned above. Then, the goal is to evaluate that the radiological evaluation of knee arthritis enriched with a kinematic measurement at the knee, correlates better with the function score.

In the first part of the work, a method to compute the pseudo-kinematic of the femoro-tibial joint was developed. Five flexion angles of the knee were imaged with the low dose biplane EOS® system (0, 15, 30, 45 and 70°); the pair of radiographs of the knee in full extension allowed the 3D reconstruction of the femur, the tibia and the fibula. After the segmentation of the bony contours on every radiograph, the registration of the geometric models of the bony structures allowed to extract the position of the tibia relatively to the femur, from one flexion angle to the following one. The intrinsic 2D/3D registration was based on the iterative closest point algorithm. In addition, the introduction of both a weighted registration and the registration of the tibia/fibula as a rigid body considerably reduced the errors of the axial rotations computation. After the validation of the method on ten subjects, its measurement error in reproducibility and precision was assessed and lower than 0.4mm and 0.5° and the method sensitivity to the 3D reconstruction was lower than 1.7mm and 1.4°. With a study of the pseudo-kinematic adduction (ADD) by a clustering analysis, this parameter was found to be a functional radiological marker of the knee arthritis. The asymptomatic subjects were in mild ADD (<5°) or in abduction (ABD) whereas the arthritic subjects were is severe ADD >5° (14 patients with internal knee arthritis, 14 asymptomatic subjects). Considering knee ADD as a risk factor for the occurrence and progression the knee arthritis, it was possible to compute composite Kellgren-Lawrence grades for 28 subjects: the original Kellgren-Lawrence grade became worsened (higher) with severe knee ADD and decreased (lower) with knee ABD. The correlation of the KOOS (Knee Ostearthritis Outcome Score) function scores with these composite grades was lightly but not significantly higher than the correlation with the original Kellgren-Lawrence grades. In the last part of the work, the pseudo-kinematic screw-home mechanism was analysed (7 patients with internal knee arthritis, 5 asymptomatic subjects). The asymptomatic tibias were in external rotation (mean value 9°), the arthritic tibias were in a milder external rotation (0-5°). Moreover, for an asymptomatic subject, a low ratio of the sagittal slopes of the tibial plateaus and a low sagittal slope of the medial plateau were correlated with the internal tibial rotation. Also, results suggested the coronal slopes of the tibial plateaus had an influence on the external tibial rotation.

This doctoral work provided a technical solution to evaluate the 3D kinematic of asymptomatic and arthritic knees with the EOS® system, with high reproducibility and accuracy. Also, knee adduction was found to be a functional radiological marker of knee arthritis and the necessity of complementing the radiological evaluation of the knee arthritis with such a measure was confirmed. Finally, the analysis of the knee morphology with the knee function revealed new hypotheses to a better comprehension of the joint, to a more complete evaluation of the pathology.
